What to send
- The public page URL and the exact fact you are challenging.
- What you believe is accurate and why it matters.
- A current primary source, official record, or provider-controlled policy page.
- Your relationship to the organization or fact, if relevant.
What happens next
- We record the request and preserve the current source trail.
- A reviewer compares the challenge with the available evidence.
- We correct, clarify, label a conflict, mark the information unknown, or explain why the published fact remains.
- Accepted changes retain their source and checked date.
Provider authority
A provider can request factual review without proving organization authority. Authority is required only for organization-account access or a submission that claims to speak for the organization. It never grants direct publishing control.
